ANN ARBOR — Jewish Family Services of Washtenaw County has received a $25,000 Community Health Matching Grant from the Blue Cross Blue Shield of Michigan Foundation.

The funding will support a new pilot program bringing occupational therapy and nutrition counseling directly to vulnerable older adults in Washtenaw County who want to age safely and independently at home.

The program will serve about 25 seniors facing barriers such as transportation and limited access to specialized care. Services will include in-home safety assessments, support with daily living activities, and nutrition therapy for chronic conditions.

Jewish Family Services officials say the pilot also includes development of a sustainable billing model to pursue Medicaid reimbursement, allowing the program to continue beyond the grant period.

Jewish Family Services of Washtenaw County serves more than 21,000 people each year across the region.
